To be eligible, you need to:

• Be 65 years of age or older
• Be enrolled already in Medicare
• Live in the service area. Currently serving Alachua, Brevard, Broward, Citrus, Dade, Duval, Hernando, Hillsborough, Lake, Manatee, Marion, Orange, Osceola, Palm Beach, Pasco, Pinellas, Polk, Sarasota, Seminole and Volusia counties.
• Meet the clinical eligibility requirements of needing help with daily living activities (like bathing, dressing, walking, or help with dementia)
• Meet Medicaid financial eligibility requirements for this care level
